How to get birth certificate in India

Which form to fill?

Official form: It’s just a specimen of the form that parents’ fill within 21 days of the birth occurrence. The applicant’s parents fill it.

Official form’s requirement:

Which documents are to attest and attach?

For official form’s attachment: The registrar or the sub-registrar will declare it only when he’ll get the attested photocopy of the original proofs. But, the original copies should be enclosed for verification only.
